Welborn Health Plans

On November 1, 2003, USHH entered into a long-term HMO management and
operational outsourcing agreement with the Welborn Clinic (Evansville, IN) to
manage all aspects of its 36,000+ member HMO, Welborn Health Plans (WHP).  
 Under this agreement, USHH will be responsible for all day-to-day operations
and management, including claims, enrollment, member services, marketing/sales/
advertising, underwriting, medical management, quality assurance, provider
contracting/service, information systems, pharmacy management, data reporting/
analysis, and regulatory, legal and financial management.

On May 31, 2006, Neighborhood Health Providers of Indiana, LLC (NHPI)
purchased WHP from the Welborn Clinic.  Upon the sale of WHP, NHPI entered
into a long-term management agreement with USHH to continue in the same
capacity as its relationship with the Welborn Clinic.


Other Clients

On November 15, 2004, USHH entered into a long-term HMO management and
operational outsourcing agreement with a provider-sponsored start-up
commercial, Medicare, and Medicaid HMO based in St. Petersburg, Florida.  
Under this agreement, USHH is responsible for most day-to-day operations,
including claims, enrollment, member services, utilization management, quality
assurance, underwriting, provider service, data reporting/analysis, premium billing
and accounts payable.  USHH has helped this client grow to over 13,000
members through its technology and operational services.

USHH continues its amazing growth throughout 2007.  We are in discussions with
several large and start-up HMOs, PSNs and Medicare Special Needs Programs
(SNPs) to provide full management and operational support to these entities.
